Adding forms(reports) in mobotour

Forms are often referred to as reports.   This allows you to replicate your paper reports so they can be submitted digitally from a mobile device.  
  1. Log into the portal with your account username and password
  2. Navigate to the ’Forms’ link on the left under ‘Configuration’
  3. Click ‘Add’ at the top right of the screen
  4. Fill in all required fields which have a red asterisk to the right
  5. Fill in advanced options Advanced Options
  6. Click the 'Submit' button



  • Form Name = Shows on Reports
  • No. of Fields = how  many fields will this report have.  Once you select a number, more fields will display.  For example if I entered ‘4’ fields I would be required to fill out data about these fields 
  •       Field Name = the value the workforce will see on  the mobile device and will show on reports
  •       Field Type = do you want this be text, drop downs, check boxes, read only, etc
  •       Account = filled in for you


  • Tags = for reporting data segmentation
  • Shift = deprecated functionality .. won’t be supported in future releases
  • Personnel = mobile device segmentation
  • Activate = will this display to mobile devices
  • Advanced options - this will allow additional functionality to happen, like immediate notification of submission, scans, video, and audio.  (click here to read about additional features)
